How to prevent the system using the same account and IC number

Hi
My only problem here is to not make a duplicate account number with different detail. How can i do that ?

My only problem here is to not make a duplicate account number with different detail. How can i do that ?
Now that I've been thru your code, I can't see where this duplicate stuff is happening. How does one reproduce the problem?
