Header Documentation?

So my university professor wants us to use a header documentation to display all our student information, and stuff about the program like main variables and stuff.I'm just slightly confused because when he showed us an example, it was all in a box sort of made up of //
So I'm just a bit confused on how to make that.

Is your question about actually typing the flower box or the contents of the flower box?
The contents, of course, are far more important.
